Consumer Rate Comment Templates

Health insurance companies in New York State must ask for permission from the Department of Financial Services (DFS) to change their rates. DFS considers public comments when conducting their review. There are two templates below, one for responding to a proposed increase and one for responding to a proposed decrease.
Once you download the template and add your own information, you can copy and paste your comment into the form provided by DFS: https://myportal.dfs.ny.gov/web/prior-approval/submit-a-comment.
